Sports re-certification set for Spencerport
All students wishing to participate in any fall, winter and/or spring interscholastic sport for the 2005-2006 school year must have proof of a valid sport physical and sport re-certification. The physical is to be done through the family physician. A completed SCS Health Appraisal form, signed by the family physician, must be returned to the school nurse at re-certification. All fall athletes must attend the New York state mandated re-certification. Bring proof of private physical completed by your physician if obtained during the summer, and re-certification form completed and signed by parent. Physical forms are available on the school website as well as in the high school and middle school main offices www.spencerportschools.org. Re-certification dates are:
